Application Forms 2007 Faces of the Future Participation Form To take part in this year’s Faces of the Future survey, ACT must receive orders no later than September 28, 2007. Surveys must be returned for scanning no later than November 30, 2007. ACT will ship survey forms and provide clear instructions on how to administer them. ACT will scan the completed surveys and provide a comprehensive reporting package including summary and graphics reports for credit and noncredit students, a national comparison data report, and a data CD. Summary reports and data CDs will be shipped three to four weeks after completed surveys are received by ACT. Graphics and national comparison reports will be shipped in early March, 2008.
